The medics treat a man who collapsed after inflicting a torrent of insults on his colleagues, and their investigations uncover that he has frontal lobe inhibition, causing him to vocalise his innermost thoughts. The patient's condition proves to be amusing, until his harsh truths threaten to destroy his relationship with his wife and daughter. Meanwhile, House suspects Wilson is keeping secrets from him.
